Position Description:
The Nurse Practitioner, employed by APMHA, provides advanced nursing with a key focus on mental health, AOD (Alcohol or Other Drugs) and physical health screening, assessment, treatment, and referral. In addition, the Nurse Practitioner will bring professional leadership and guidance to the team, as well as providing biopsychosocial advice for mental health staff within the Local, contributing to their education, training and professional development.
This role makes up part of the Towards Change Team and is a crucial part of the Victorian Mental health reform work that the Local partners are undertaking. This role will be part of a growing service system that places the community in the centre of the Local Mental Health and Wellbeing network across Victoria.
The Towards Change Team provide a range of assessments depending on client need and will deliver evidence-based interventions beyond mental health, extending to physical health, substance dependence issues, employment, and relationships support, as part of the broader Local workforce.
The Towards Change Team are responsible for providing assessment, counselling and care planning for clients referred via the Support and Connect Program, and will work alongside dedicated Wellbeing Peer Navigators from the Support and Connect Team.

Candidates will need:
- Current AHPRA Registration with endorsement as a Nurse Practitioner.
- Experience working in the mental health &/or AOD sectors (minimum 4 years).
- Demonstrated time management abilities, excellent communication (written and oral) and interpersonal skills.
- Demonstrate a friendly, proactive approach and ability to work with competing demands and balance priorities with efficiency and professionalism.
- Ability to work effectively independently and within a team.
- Certification of qualifications
- National Police Check
- International Police (if required)

The Towards Change Team clinician, employed by APMHA and working with Peer Navigators from the Support and Connect Team, will provide evidence based clinical interventions for participants. The Towards Change Team will encompass clinically and therapeutically proficient clinicians, as well as a Creative Arts Peer Worker, providing tailored mental health support to clients and their families via their wellbeing plan, including to those with co-occurring substance use or addiction.

Mental Health Clinician - North West Melbourne Region
Position: Mental Health Clinician (MH Social Worker, MH Occupational Therapist, Cred Mental Health Nurse or Psychologist) – (or working towards mental health accreditation).
Role: Subcontractor or Employment
Location: Will depend on allocated Residential Aged Care Facilities. Region for this program includes LGAs of Brimbank, Hobsons Bay, Maribyrnong, Melton, Moorabool, Wyndham, Moonee Valley
Remuneration – Employee:
- Above award remuneration, hourly rate will reflect experience and skills
- Salary packaging (up to $15,900 package and $2,650 in meals/entertainment benefits)
- Superannuation of 11.5 % (increasing to 12% in July 2025)
Remuneration – Subcontractor:
- $140 per billable session (exclusive of GST) paid monthly in arrears
- Face to Face/Onsite. No Telehealth
- 1 hour capacity building per facility per BAS quarter

You will ideally possess:
- Relevant tertiary qualifications in Occupational Therapy, Mental Health Nurse, Social Work or Psychology (Mandatory) and registration with AHPRA/AASW.
- Must have mental health endorsements, or eligible to apply for same.
- Experience working with people with mental illness across a broad range of presentation.
- High levels of emotional intelligence and the ability to engage and work with multidisciplinary teams and other stakeholders.
- Ability to work with general practice, mental health services and program staff.
- Sound IT skills to enable full application of APMHAs virtual systems including client management system experience.
- COVID, Influenza vaccination are required for working within a Residential Aged Care facility.
