APMHA’s Relapse Prevention and Discharge Program (RP&D) was developed to enhance client outcomes by providing continuous support throughout the mental health recovery journey. Recognising that recovery extends beyond the duration of formal counselling, APMHA aimed to bridge the gap between service discharge and sustained wellbeing.
The program was co-designed with input from clinicians and a Consumer Lead, ensuring it reflects the lived experiences of clients. It offers a structured approach that includes pre-discharge planning and post-discharge follow-up, empowering clients to manage their mental health proactively.
By focusing on early intervention, personalised care, and ongoing support, the RP&D program embodies APMHA’s commitment to improving services and minimising service gaps, ultimately fostering a more resilient and connected community.
This Pilot Program commenced in the Goulburn Valley & North East Vic regions of Murray PHN in April 2024 and has now been extended to the South Western Sydney PHN region.
